I need help finding a good bid sheet for bidding welding job....

For those ignorant like me of not knowing what a bid sheet is, please explain:
Things like?
1)Price of materials
2)Your hourly wage
3)Your experience of that kind of job
4)Gas to and from job and runs to the different stores
5)Cost of gas and consumables
6)What your competition charges (depending where you live)

Its like an RFP ,I needed a proffessional looking way to submit my charges to a company.I could probally make -up something ,but not a computer whiz...100% welder here no good at tha paperwork that goes with it.....

Here's an example of what I use. Now mind you, I'm not a professional welder so I don't know if there's something you're supposed to do, but this is how I do it for metal, or concrete or computer work, etc.
Miller Maxstar 200 DX

Yes,thats a simmilar draft like I was looking for.I think I found one today, with a little touch up i think will be perfect ,but always good to see others.Again thank you for your time......Youll have to resize to read it, i could nt figure that part out.....

This is our residential estimate form (three parts) 1 for customer. 1 for office file and 1 for our estimators file. Again I'm not a computer pro but I know that our computer (office nerd) can cross out our name so you can just copy your info where ours is. Maybe he can even put yours in?
